Job summary

Optum is seeking a Home Health Patient Care Manager in Connecticut to supervise an interdisciplinary team delivering high-quality home health services in accordance with physician plans of care and state/federal regulations. You will coordinate referrals, clinician assignments, and ongoing insurance approvals while ensuring tailored patient care across diverse settings.

Requirements include an unrestricted RN license, a valid driver's license, and access to reliable transportation.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Provides clinical services within the scope of practice, per physician plan of care, and in coordination with the health care team.
• Receives referrals, assigns clinicians, evaluates orders, and plans start of care visits.
• Coordinates determination of patient benefits, medical necessity, and ongoing insurance approvals.
• Ensures patient needs are continuously assessed and care is individualized and compliant with physician orders.
• Reviews assessments and plans of care daily, with clinician consultations as appropriate.
• Functioning respectfully in any home setting regardless of age, race, creed, color, sex, disability, or financial condition.
