Job summary

UnitedHealth Group seeks a Registered Nurse to provide and direct nursing care to patients in their homes, coordinating total plans of care with physicians and care teams. Coverage includes Hartford and surrounding towns.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.

Responsibilities

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient/client's care team from admission to discharge.
• Complete clinical nursing assessments per program requirements and payer needs.
• Ensure patient/client eligibility and medical necessity for services as defined by payer and agency policy.
• Develop and revise individualized plans of care/service plans with other providers.
• Ensure plan of care frequency and duration meets patient needs and obtain physician approval for revisions.
• Ability to work in diverse home situations.
