Job summary

UnitedHealth Group is seeking a Registered Nurse to deliver and direct home health nursing care across Hartford and surrounding towns. You will coordinate total plan of care with physicians and the care team, assess patient needs, ensure eligibility and medical necessity, and support families in planning and exchanging information.

In this role you’ll provide high-quality clinical services within infection control standards, work independently, and participate in payer-driven program

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a reliable vehicle or public transportation.
• Preferred CPR certification or ability to obtain within 90 days.
• 1+ year of RN experience and ability to work independently.

Responsibilities

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient’s care team from admission to discharge.
• Complete clinical nursing assessments per program requirements and payer needs.
• Ensure patient eligibility and medical necessity for services as defined by payer and policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency and duration meet patient needs and revise with physician approval.
• Function in diverse home settings regardless of patient demographics.
