Qualifications

• Active RN license in the state of practice.

Responsibilities

• Conduct thorough assessments to ensure personalized, comprehensive home care.
• Educate patients and families on health maintenance and disease prevention.
• Assist with ADLs with dignity and respect.
• Coordinate scheduling with patients according to plan of care and preferences.
• Monitor patient status and report changes to care team promptly.
• Administer medications and treatments as prescribed by physicians.
• Communicate with case managers and treating physicians for unified care.
• Maintain accurate, heartfelt EMR documentation of patient visits.
• Provide emotional support to patients and families, reinforcing trust.
