Qualifications

• Valid and active RN License in the state of practice.
• Willingness to travel directly to patients' homes.
• Malpractice Insurance required.

Responsibilities

• Conduct thorough patient assessments for personalized care.
• Guide patients and families on health maintenance and disease prevention.
• Provide assistance with Activities of Daily Living (ADLs).
• Monitor and report changes in patient symptoms or behavior.
• Administer medications and treatments as prescribed.
• Document patient care accurately in the EMR system.
