Job summary

Interim HealthCare Inc. in Kentucky seeks a Registered Nurse Case Manager (RNCM) to perform in-person interviews, assess patient needs, and oversee care plans with home health aides, LPNs, RNs, and other caregivers.

You will educate patients and families on home health procedures, promote preventative care to reduce readmissions, and collaborate with providers to adjust care as needs evolve. The role offers competitive pay, full-time hours, and opportunities to advance within a patient-centered

Qualifications

• Current unrestricted RN license in the state of Kentucky/position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to care sites
• At least 1 year RN experience in home health or similar setting; case management/utilization review experience preferred
• OASIS experience preferred
• Trach/vent experience preferred but not required

Responsibilities

• Conduct in-person patient interviews and comprehensive physical assessments
• Oversee implementation and ongoing assessment of patient care plans with home health aides and nurses
• Communicate patient conditions and collaborate with providers to deliver care as needs evolve
• Educate patients and families on home health care procedures (wound care, IV administration, medication management)
• Work to decrease readmissions by promoting preventative care and continuity of care
