Job summary

The Christ Hospital Home Health in Cincinnati, OH seeks an experienced Registered Nurse to provide comprehensive skilled care in patients' homes. You will educate patients and families, document accurately, and ensure compliance with state laws and agency policies.

The RN will administer medications, perform OASIS assessments, develop and adjust care plans, collaborate with LPNs and aides, participate in on-call rotations, and maintain licensure while delivering high-quality, compassionate care.

Qualifications

• Minimum of one year of RN experience in an acute care setting; home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Ability to remain calm, compassionate and patient while providing care.

Responsibilities

• Provides skilled nursing care in patients' homes as ordered by the physician.
• Observes and monitors patient conditions and documents changes.
• Performs OASIS assessments to tailor an individualized plan of care.
• Administers medications as prescribed by the physician.
• Educates patients and families on disease processes and plan of care.
• Oversees and coordinates care with Aides and LPNs; participates in on-call rotations.
