Optum Care at Home in Washington State seeks a Field RN Case Manager to support Medicaid LTSS/DSNP programs across WA and CO. Role involves in-home visits (~75%) and telephonic management (~25%), with emphasis on care coordination and patient education.

Qualifications

• Active unrestricted RN licensure in state of residence and state of assignment, compact licensure or ability to obtain.
• Minimum 2+ years of experience as a Registered Nurse.
• Experience with Medicaid LTSS, LTSS, HCBS or similar programs.
• Proven ability to work with diverse care teams in various settings, including patients’ homes.
• Solid computer skills including use of Electronic Medical Records.

Responsabilités

• Conduct in-home assessments, care coordination visits and community-based engagement roughly 75% of time in Washington.
• Provide telephonic case management for WA and CO (approx. 25%).
• Assess health status within licensure scope and model of care; establish goals and plans of care.
• Coordinate with interdisciplinary team to address medical, functional and social barriers to care.
• Document accurately in the EMR and educate patients and families on health maintenance and follow-up.
