Job summary

UnitedHealth Group, through Optum Home & Community Care, is seeking a Behavioral Health Case Manager - DSNP to conduct in-home assessments and field visits across Washington state, with telephonic support for members in Washington and Colorado.

The role focuses on coordinating behavioral health, medical, and social services, developing care plans, and navigating Medicaid benefits, with up to 75% travel and a strong emphasis on whole-person care.

Qualifications

• Master’s degree in social work or related behavioral health field.
• Active Washington behavioral health licensure (LICSW/LCSW/LMSW) or equivalent.
• Licensed in assigned states or able to obtain licensure by start date.
• 2+ years in a healthcare setting.
• 1+ years of clinical or case management experience.
• Experience assessing behavioral health and/or substance use conditions.
• Experience with Medicaid LTSS or related programs (HHBS, etc.).
• Ability to travel up to 75% within Washington.
• Valid driver’s license and reliable transportation.
• Ability to physically navigate home settings, move 30 pounds.

Responsibilities

• Conduct in-home and telephonic behavioral health assessments.
• Develop and manage individualized care plans for high‑risk members.
• Coordinate behavioral health, medical, and community services.
• Review treatment requests and ensure appropriate utilization and level of care.
• Educate members on Medicaid/Medicare benefits and access to services.
• Connect members to LTSS, HCBS and community resources.
• Support transitions from hospitals or SNFs to home-based care.
• Document assessments and care plans in electronic systems.
