Job summary

LiveWell Homecare Agency, LLC is seeking a registered nurse to perform in-home assessments, educate patients, and coordinate care in a community-based setting.

The role involves evaluating health status, identifying barriers to treatment, making referrals, and supporting patient self-management. You will maintain clear communication with members through in-person visits or phone calls to ensure continuous, high-quality care.

Qualifications

• Active RN license in Washington state.
• BLS certification or ability to obtain.
• Minimum 1 year of clinical and home care experience.

Qualifications & Skills:

• Unrestricted RN license in Washington
• Basic Life Support certification or ability to obtain
• At least 1 year of clinical and home care experience
• Strong computer skills, customer service, patient-focused attitude, and adaptability
• Valid driver’s license and reliable transportation

Preferred Qualifications:

• BSN or CCM certification
• Experience in discharge planning, utilization review, or risk management
• Telephonic care experience and bilingual in English/Spanish
• Proficiency in MS Office and managed care background
