Job summary

Optum is seeking a Registered Nurse to join our primary care team serving the Lewis County area in Washington. This role supports APC/ISNP/IESNP care plans and collaborates with providers, delivering care telephonically and in home settings. The RN works under provider direction within CPM models of care.

Required: WA RN license, 3+ years RN, 1+ year case management, EMR experience, and strong communication. 75% local travel; $7,500 sign-on bonus for external candidates.

Qualifications

• Current unrestricted RN license in Washington state.
• 3+ years of RN experience.
• 1+ year of case management experience.
• Experience with electronic medical records.
• Basic Microsoft Office skills.
• Proven communication and customer service skills.
• Valid driver's license and ability to travel locally 75%.

Responsibilities

• Establishes and maintains collaborative relationships with ICT, member/RP and primary caregiver staff in the ALC and APC.
• Coordinates with utilization review, provider relations liaison, pharmacy and other units for seamless care.
• Reviews member status and progress toward ICP goals; adjusts ICP as needed.
• Performs administrative functions for PCP and APC as directed.
• Participates in program effectiveness analysis and planning.
• Performs member visits and rounding as directed by the APC.
• Communicates member status updates to the APC and ICP.
• Delivers clinical education programs for facility staff.
• Maintains HIPAA-compliant handling of confidential information.
