Job summary

Professional Case Management in Georgia is seeking RN case managers to conduct in-home nursing visits for former Nuclear Weapons Workers with chronic and terminal illnesses. You will schedule your own hours and provide direct care and case management with weekly visits.

The role requires an active RN license, BSN preferred, CPR certification, and at least two years of nursing experience including one year in home care. Supervisory/case management experience is preferred.

Qualifications

• RN licensed nurse
• BSN preferred
• Current, unrestricted RN license in state of practice
• 2+ years nursing experience including 1 year in home care
• 1 year supervisory/case management experience preferred
• Current CPR certification

Responsibilities

• Provide direct care and case management for assigned clients
• Coordinate with regional director and staff to improve services
• Plan, implement, and evaluate care provided
• Participate in client care conferences and staffing coordination
• Ensure home care needs are met and nursing charts are complete
• Review clinical documentation for accuracy and completeness
• Participate in agency performance improvement initiatives
