Job summary

Professional Case Management is seeking RN Case Managers to provide in-home nursing visits for former Nuclear Weapons Workers with chronic or terminal illnesses in Georgia. You will assess, support, and coordinate care, ensuring home safety and medication planning while maintaining thorough documentation.

Applicants should have a state RN license (BSN preferred) and at least two years of nursing experience, including one year in home care.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Maintain and coordinate home care plans with regional director and staff.
• Ensure timely charting and accurate documentation of patient visits.
