Job summary

Professional Case Management is hiring Registered Nurse (RN) Case Managers to provide in-home nursing visits for former Nuclear Weapons Workers with chronic/terminal illnesses in Georgia. You will perform weekly visits, conduct vitals, assess home safety, manage medications, and supervise care teams to ensure high-quality outcomes.

The role supports flexible scheduling, competitive pay at $42/hour, and full benefits including PTO, medical/dental/vision coverage, and a 401k.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care; participate in client care conferences as needed.
• Serve as a local on-site clinical resource and support staffing coordination.
• Adhere to nursing delegation guidelines and agency policies.
• Participate in performance improvement initiatives and chart reviews.
• Perform supervisory visits and review nurse shift reports for quality and safety.
• Assist with marketing activities and community outreach as requested.
