Job summary

Professional Case Management is hiring RN-Case Managers in Georgia to conduct in-home nursing visits with former nuclear workers who have chronic illnesses. Visit durations are typically 1–2 hours weekly, performing vitals, safety checks, and medication planning while coordinating with caregivers and the regional director to optimize client care.

Qualifications

• Graduate of an approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state of practice.
• Minimum of two years nursing experience including one year in home care.
• One year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide in-home nursing visits weekly, including vitals and assessments.
• Ensure home safety with medical equipment and supplies.
• Fill weekly medication planners and oversee caregivers providing care at home.
• Collaborate with regional director and staff to identify and improve services.
• Plan, coordinate, and participate in client care conferences as needed.
• Serve as a clinical resource and ensure client home care needs are met.
