Professional Case Management is seeking a registered nurse to perform in-home case management for former Nuclear Weapons Workers with chronic and terminal illnesses in South Carolina. The role involves weekly visits, 1–2 hours each, and comprehensive assessments including vitals, safety checks, and medication planning.

You will supervise caregivers and coordinate with the regional team. The ideal candidate has an active RN license, 2+ years of nursing experience (1 in home care), and CPR

Qualifications

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsabilités

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care provided; participate in client care conferences as needed.
• Coordinate with regional director and personnel to identify and correct issues.
• Supervise and support clinical personnel; ensure adherence to nursing delegation guidelines.
• Ensure timely completion of clinical documentation and chart reviews.
• Engage in marketing activities such as visiting clients or physicians to discuss agency programs.
