Job summary

Professional Case Management in South Carolina seeks RN Case Managers to provide in-home nursing visits for former nuclear workers with chronic or terminal illnesses. You will conduct vitals, home safety checks, and coordinate both skilled and non-skilled caregivers. A strong emphasis on direct patient care and documentation is required.

You must hold an active RN license, have 2+ years nursing experience (1+ in home care), and prefer BSN. CPR certification is required.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care provided; participate in client care conferences.
• Collaborate with regional director and staff to identify and correct issues and improve services.
• Adhere to Agency policies and perform supervisory visits as needed.
• Participate in performance improvement initiatives and maintain clinical documentation.
