Role emphasizes flexible scheduling and work-life balance. RN licensure in the state of practice and CPR certification are required, with BSN preferred and prior home health experience desirable.

Qualifications

• RN license in the state(s) of practice is required.
• CPR certification is required.
• Home health or related field experience preferred.
• Supervisory or case management experience is preferred.

Responsibilities

• Provide direct care and case management of assigned clients.
• Plan, implement and evaluate care; coordinate client care conferences.
• Serve as on-site clinical resource and support regional director and staff.
