Job summary

Right at Home Boston Northwest is seeking an experienced Registered Nurse (RN) Case Manager for a field-based home care role that serves clients across the Boston Northwest service area. You will perform in-home assessments, coordinate plans of care, and provide ongoing clinical management while traveling locally to client homes.

Qualifications

• Active, unrestricted MA RN license.
• Minimum 2 years of nursing experience.
• Experience in home care, geriatrics, private-duty nursing, or case-management preferred.

Responsibilities

• Conduct in-home nursing assessments and reassessments for new and existing clients.
• Develop, review, and update individualized plans of care.
• Provide ongoing clinical case management and follow-up visits.
• Monitor changes in client condition and coordinate appropriate clinical follow-up.
• Educate, coach, and support caregivers in delivering safe, high-quality care.
• Serve as a clinical resource for clients, families, caregivers, and the office team.
• Collaborate with the Director of Nursing, scheduling, and care management teams to maintain continuity of care.
• Communicate with physicians and other healthcare professionals when appropriate.
• Support clients and families during changes in care needs and transitions.
• Maintain accurate and timely clinical documentation.
• Build positive relationships with clients, families, caregivers, and community partners.
