Right at Home Boston Northwest is seeking a compassionate Registered Nurse (RN) / Case Manager to join our Home Care team serving Bedford, MA and surrounding communities. You will coordinate high-quality in-home care, support caregivers, and help clients stay safe and comfortable at home.

Responsibilities include conducting in-home assessments, developing care plans, and providing ongoing clinical management with timely documentation.

Competenze

• Active, unrestricted Registered Nurse license in Massachusetts.
• Two or more years of nursing experience; home care, geriatrics, private-duty care, or care-management experience preferred.
• Strong clinical judgment, communication, organization, and documentation skills.

Mansioni

• Conduct in-home assessments and develop/update individualized care plans.
• Provide ongoing clinical case management, follow-up visits, and timely documentation.
• Support and educate caregivers, respond to condition changes, and ensure care plans are followed.
• Serve as a clinical liaison for clients, families, caregivers, physicians, and other professionals.
• Collaborate with Nursing Director, scheduling, and care management teams to support continuity of care.
• Coordinate clinical follow-up and address client/family concerns.
