Job summary

Right at Home Boston Northwest is seeking an experienced Registered Nurse (RN) Case Manager to join our field-based home care team serving the Boston Northwest area, including Bedford, Lexington, Burlington, Woburn and surrounding communities.

You will conduct in-home assessments, develop plans of care, coordinate care with families and clinicians, and educate caregivers while enjoying professional independence and meaningful relationships with clients.

Qualifications

• Active, unrestricted MA RN license.
• 2+ years nursing experience.
• Home care or case-management experience preferred.
• Strong clinical assessment and judgment.
• Excellent communication and interpersonal skills.
• Strong organization, time-management and documentation skills.
• Comfortable working independently within a team.
• Compassionate and dependable with older adults and families.
• Valid driver's license and insured vehicle.
• Ability and willingness to travel locally between clients' homes.

Responsibilities

• Conduct in-home nursing assessments and reassessments for new and existing clients.
• Develop, review, and update individualized plans of care.
• Provide ongoing clinical case management and follow-up visits.
• Monitor changes in client condition and coordinate appropriate clinical follow-up.
• Educate, coach, and support caregivers in delivering safe, high-quality care.
• Serve as a clinical resource for clients, families, caregivers, and office team.
• Collaborate with Director of Nursing, scheduling, and care management teams to maintain continuity of care.
• Communicate with physicians and other healthcare professionals as appropriate.
• Support clients and families during changes in care needs and transitions.
• Maintain accurate and timely clinical documentation.
• Build positive relationships with clients, families, caregivers, and community partners.
