Job summary

Right at Home Boston Northwest is seeking an experienced Registered Nurse (RN) Case Manager to join our home care team. You will conduct in-home assessments, develop plans of care, and coordinate services for older adults in the Boston Northwest area, including Burlington and surrounding communities.

The role emphasizes clinical assessment, independent judgment, and collaboration with families while maintaining strong ties to our local team.

Qualifications

• Active, unrestricted Registered Nurse license in Massachusetts.
• 2+ years of nursing experience.
• Experience in home care, geriatrics, private-duty nursing, or case-management preferred.
• Strong clinical assessment and judgment skills.

Responsibilities

• Conduct in-home nursing assessments and reassessments for clients.
• Develop, review, and update individualized plans of care.
• Provide ongoing clinical case management and follow-up visits.
• Monitor changes in client condition and coordinate clinical follow-up.
• Educate, coach, and support caregivers in delivering safe, high-quality care.
• Serve as a clinical resource for clients, families, and the care team.
• Collaborate with nursing leadership and care teams to maintain continuity of care.
• Communicate with physicians and other healthcare professionals as needed.
• Support clients and families during changes in care needs and transitions.
• Maintain accurate and timely clinical documentation.
• Build positive relationships with clients, families, caregivers, and community partners.
