Job summary

Always Family HomeCare seeks an experienced RN Care Manager to work PRN and provide field support for seniors in Virginia. You will visit clients at home, assess needs, write care plans, and coordinate changes in care with providers, reporting to the Administrator.

The role emphasizes compassionate communication, travel to client homes, and precise documentation in EMR systems. Requires VA RN license, CCM/CMC or ability to obtain, and US work authorization with background/drug checks.

Qualifications

• Virginia RN license active and in good standing.
• CMC/CCM certification or ability to obtain.
• Experience in Homecare, Assisted Living, or Skilled Nursing desirable.
• Strong communication across phone, email, Zoom, telehealth.
• Authorized to work in the United States; consent to background check and drug screen.

Responsibilities

• Care management for seniors to ensure best quality of life.
• Accompany clients to appointments; coordinate care among providers.
• Advise on care options and connect with appropriate providers.
• Maintain thorough documentation of client cases.
• Provide in-home assessments if needed.
