Home Instead is seeking a Registered Nurse (RN) to serve as a Client Care Coordinator in the Charleston area. This field-based role blends nursing knowledge with care coordination, relationship management, and consultative guidance to support families seeking non-medical home care for seniors.

The ideal candidate will hold an active SC RN license, have 3+ years in nursing or care coordination, and be comfortable traveling throughout the Charleston service area.

Aufgaben

• Conduct in-home care consultations and assessments with prospective clients and families.
• Develop individualized non-medical care plans based on client needs and preferences.
• Educate families to help them decide on care for aging loved ones.
• Build trust with families and guide qualified prospects to start services.
• Maintain high conversion from consultations to new clients.
• Complete service agreements, care plans, and required documentation.
• Coordinate with Scheduling and Client Care for timely starts of care.
• Perform ongoing Quality Assurance visits with existing clients and families.
• Identify opportunities to expand care when beneficial to safety or quality of life.
• Proactively address client concerns and document in company systems.
• Participate in weekend on-call rotation as backup roughly every fifth weekend.
• Represent Home Instead professionally throughout the Charleston community.
