Job summary

HarmonyCares is seeking a Nurse Care Manager to join our home-based care team. You will manage high-risk patients, coordinate with pod leaders, and develop individualized care plans. This role emphasizes transitional care, patient education, and collaboration with the broader care team to improve outcomes.

Required RN license and 2+ years in care management; BSN preferred. This is a full-time position supporting patients in/home-based settings across multiple states.

Qualifications

• Active Registered Nurse License.
• Minimum 2 years of care management in health systems.
• Strong clinical and proactive thinking.
• Effective communication skills.
• Ability to perform extensive telephone assessments.
• Knowledge of Medicare regulations and home care standards.
• Experience with group presentations and teaching/training.
• Excellent interpersonal and written/oral skills.
• Proficient with Word, Excel, PowerPoint.
• Time management to meet documentation deadlines.

Responsibilities

• Coordinate care with pod leader to provide tailored services.
• Develop and manage personalized care plans and address gaps.
• Co-chair the pod and prioritize patient needs.
• Manage transitions of care to avoid hospital admissions.
• Facilitate High Risk Huddles and ensure follow-up actions.
• Review medical records to identify care gaps.
• Update patient care plans regularly.
• Perform nursing assessments via telephone.
• Educate patients/caregivers on disease management and prevention.
• Document interactions in patient records per guidelines.
• Act as liaison between patients, providers, and resources.
