Job summary

Activate Care is seeking a qualified Registered Nurse to conduct in-person health assessments for a managed care health plan’s dually eligible members in Massachusetts. The role involves home visits, accurate documentation, and seamless handoff to care management.

Strong clinical judgment and independent work in diverse homes are essential. Requirements include active MA RN licensure, at least 1 year in home health, and a valid driver’s license with access to a vehicle.

Qualifications

• Active RN license in MA in good standing.
• Associate of Science in Nursing required; BSN preferred.
• Minimum 1 year of clinical home health experience.
• Must have a valid driver’s license and access to a personal vehicle.

Responsibilities

• Conduct in-person health assessments in members’ homes or community settings.
• Document assessments in the health plan’s care management platform (GuidingCare).
• Submit SOAP notes to Care Manager within 48 hours of assessment.
