Job summary

Qualifications

• Active RN license in the state of practice.
• Willingness to travel to patients' homes.
• Current BLS certification.
• Ability to pass a state & federal background check; professional liability required.

Responsibilities

• Perform thorough assessments to deliver personalized, comprehensive in-home care.
• Educate patients and families on health maintenance and disease prevention.
• Assist with Activities of Daily Living (ADLs) with dignity and respect.
• Coordinate schedule with patients’ plans of care and approved hours.
• Monitor patient symptoms and report changes to the care team.
• Administer medications and treatments as prescribed by the physician.
• Communicate with Case Managers and collaborating physicians to ensure unified care.
• Maintain accurate patient logs in the EMR system.
• Provide emotional support and guidance to patients and families.
