FOX Rehabilitation is seeking licensed Physical Therapist Assistants in Wilmington, DE to deliver one-on-one, in-home care under supervision of a PT. You will guide patients through individualized exercises, rebuild strength, and support independence within FOX’s geriatric house calls model.

You’ll work with an interdisciplinary team and receive mentorship to grow your clinical skills, caseload, and career in a supportive environment while delivering compassionate, hands-on care to older adults

Qualifikationen

• Valid Physical Therapist Assistant license in the state of practice (DE).
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.

Aufgaben

• Deliver individualized treatment plans established by supervising PT, adjusting activities based on patient progress.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX’s EMR system and adhere to documentation standards.
• Coordinate coverage and stay connected with patients, POAs, and referral sources.

Ausbildung
