Job summary

FOX Rehabilitation is seeking a motivated Physical Therapist Assistant to deliver one-on-one, in-home care for older adults. You will implement individualized treatment plans under supervision, guide patients through therapeutic exercises, and document visits in FOX’s EMR with meticulous accuracy.

Qualifications

• CPR certification required.
• Proficient in Microsoft Office.
• Valid PTA license in state of practice or eligibility to obtain.

Responsibilities

• Deliver individualized treatment plans established by supervising Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX’s EMR system per Clinical Documentation standards.
• Stay connected with patients, POAs, and community referral sources; assist coverage coordination.
