Job summary

FOX Rehabilitation is expanding its in-home and senior living care model in Wilmington, DE, seeking licensed Physical Therapist Assistants to deliver one-on-one plans and empower older adults to regain strength and independence. Working with supervising PT, you’ll guide therapeutic activities and document progress in FOX’s EMR, while coordinating with patients, POAs, and referral sources.

Eligible candidates enjoy mentorship, flexible scheduling, and a collaborative, interdisciplinary team

Qualifications

• Must hold a valid PTA license in practice state(s).
• Graduate of an accredited PTA program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans under supervision of a Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and activities; track progress.
• Document visits accurately in FOX’s EMR system following clinical standards.
• Coordinate with patients, POAs, and community referrals; help coordinate coverage when needed.
