Job summary

Fox Rehabilitation is expanding its Geriatric House Calls program in Shelton, CT, seeking licensed Physical Therapist Assistants to deliver personalized in-home care. You will work with a supervising PT to implement treatment plans, guide patients through exercises, and document progress in FOX’s EMR system.

This role offers extensive one-on-one patient time, mentorship, and opportunities to grow within a team-based, interdisciplinary setting while building lasting patient relationships in home

Qualifications

• Valid Physical Therapist Assistant license in CT.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office

Responsibilities

• Deliver individualized treatment plans established by your supervising Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately and on time within FOX’s EMR system.
• Stay connected with patients, POAs, and community referral sources, and help coordinate coverage when needed
