Job summary

Sierra Home Health and Hospice seeks a Physical Therapist Assistant to join a compassionate home health team. You will deliver in-home physical therapy services under the supervision of a licensed Physical Therapist, with flexible scheduling and strong leadership support.

Travel to Truth or Consequences 1–2 days per week with mileage reimbursement; you will implement treatment plans to restore mobility and function.

Qualifications

• Current licensure as a Physical Therapist Assistant in New Mexico.
• Home health experience preferred but not required.
• Strong interpersonal and communication skills.
• Confidence working independently in the field.
• Adaptability to changing schedules and home environments.
• Commitment to quality, safety, and patient-centered care.
• Reliable transportation and valid driver's license.

Responsibilities

• Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist.
• Willingness to travel to Truth or Consequences one to two days a week with mileage compensation.
• Carry out treatment plans to help patients regain mobility, function, and independence.
• Communicate progress and changes in patient condition to the supervising therapist.
• Educate patients and caregivers on exercises and safety practices.
• Document visits and progress notes accurately and on time.
• Participate in care team meetings and interdisciplinary planning.
• Uphold infection control and safety standards during all home visits.
• Maintain professional, compassionate rapport with patients and families.
