Job summary

Sierra Home Health in Truth or Consequences is seeking a Physical Therapist Assistant (PTA) to join our compassionate home health team. You will deliver in-home PT services under supervision, with travel 1–2 days per week and mileage reimbursement.

• Travel to Truth or Consequences 1–2 days per week with mileage reimbursement.
• Carry out treatment plans to help patients regain mobility and independence.
• Communicate progress and changes in patient condition to the supervising therapist.
• Educate patients and caregivers on exercises and safety practices.
• Document visits and progress notes accurately and on time.
• Participate in care team meetings and interdisciplinary planning.
• Uphold infection control and safety standards during home visits.
• Maintain professional rapport with patients and families.

Key Responsibilities

• Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist
• Willingness to travel to Truth or Consequences one to two days a week with mileage compensation
• Carry out treatment plans to help patients regain mobility, function, and independence
• Communicate progress and changes in patient condition to the supervising therapist
• Educate patients and caregivers on exercises and safety practices
• Document visits and progress notes accurately and on time
• Participate in care team meetings and interdisciplinary planning
• Uphold infection control and safety standards during all home visits
• Maintain professional, compassionate rapport with patients and families

Requirements

What We’re Looking For:

• Current licensure as a Physical Therapist Assistant in New Mexico
• Previous experience in home health preferred, but not requiredStrong interpersonal and communication skills
• Confidence working independently in the field
• Adaptability to changing schedules and home environments
• Commitment to quality, safety, and patient‑centered care
• Reliable transportation and valid driver’s license
