Job summary

Professional Case Management in New Mexico is seeking a Physical Therapy Assistant to provide in-home visits for clients. Visits are weekly and typically 1–2 hours per session.

We offer flexible scheduling and a strong work/life balance. Pay is $38 per hour, including drive time, with benefits available. Licensure as a PTA and at least 6 months of experience are required, along with a valid driver's license and clean driving record.

Qualifications

• Current license, registration or certification to practice as a Physical Therapy Assistant in the state(s) of employment or be qualified to take the PTA exam.
• Graduate of an accredited PTA program approved by the APTA.
• Minimum of six (6) months experience as a Physical Therapy Assistant.
• Demonstrated written, verbal communication skills, and interpersonal skills.
• Licensed driver with clean driving record and insured as required.

Responsibilities

• Provides physical therapy services and education/training to clients in their homes following the physician's Plan of Care and the therapy care plan.
• Completes visits in a timely manner and in accordance with agency policy.
• Documents all services provided and client progress on designated forms within seven days or per policy.
• Reports changes to the therapist or nursing supervisor on the day of observed change.
• Attends client care conferences and participates in interdisciplinary activities.
