Job summary

Sierra Home Health & Hospice seeks a compassionate Physical Therapist Assistant to join our community-based home health team serving Truth or Consequences, NM. You’ll provide in-home therapy under supervision, maintain autonomy, and support patients in regaining mobility and independence.

You’ll enjoy flexible scheduling, mileage reimbursement for travel, supportive leadership, and a mission-driven culture with CareM.

Qualifications

• Current licensure as a PTA in New Mexico.
• Home health experience preferred but not required.
• Able to travel to Truth or Consequences 1–2 days per week with mileage reimbursement.
• Strong communication and patient-centered care.

Responsibilities

• Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist.
• Willingness to travel to Truth or Consequences one to two days a week with mileage compensation.
• Carry out treatment plans to help patients regain mobility, function, and independence.
• Communicate progress and changes in patient condition to the supervising therapist.
• Educate patients and caregivers on exercises and safety practices.
• Document visits and progress notes accurately and on time.
• Participate in care team meetings and interdisciplinary planning.
• Uphold infection control and safety standards during all home visits.
• Maintain professional, compassionate rapport with patients and families.

Requirements
