Job summary

Professional Case Management is seeking a Physical Therapy Assistant to provide in-home therapy for clients in New Mexico, with a flexible schedule and the ability to manage visits weekly. The PTA will perform home visits, document progress, and coordinate with therapists and nurses to ensure quality care.

Qualified applicants have a PTA credential, at least six months of experience, and a valid driver's license. The role emphasizes safety, confidentiality, and ongoing professional development.

Qualifications

• License/registration to practice as a Physical Therapy Assistant in the employment states.
• Graduated from an accredited PTA program approved by the American Physical Therapy Association.
• Minimum of six (6) months experience as a PTA.
• Demonstrated written, verbal communication and interpersonal skills.
• Licensed driver with clean driving record and insured as required.

Responsibilities

• Provide physical therapy services and education in clients' homes per Plan of Care.
• Complete visits timely and per agency policy.
• Document services, observations, progress, and recommendations; submit within seven days or per policy.
• Report changes to the therapist or nursing supervisor on the day of change.
• Participate in interdisciplinary activities and care conferences as needed.
• Promote referrals to other disciplines and attend staff meetings and continuing education.
