Professional Case Management in Albuquerque, NM is hiring Physical Therapist Assistants to conduct in-home visits for clients. PTA visits are weekly, typically 1–2 hours, with a starting rate of $38/hour and drive-time paid.

Qualifications

• Current license, registration or eligibility to practice as a Physical Therapy Assistant.
• Graduate of an accredited PTA program approved by the American Physical Therapy Association.
• Minimum of six (6) months experience as a Physical Therapy Assistant.
• Demonstrated written, verbal communication skills, and interpersonal skills.
• Licensed driver with clean driving record and insured in accordance with state requirements.

Responsibilities

• Provides physical therapy services and education/training to clients in their homes per the Plan of Care.
• Completes visits in a timely manner per Agency policy.
• Documents all services and client progress on designated forms within seven days.
• Reports changes or schedule changes to the therapist or nursing supervisor the same day.
• Attends client care conferences and participates in interdisciplinary activities.
