Job summary

PowerBack Rehabilitation is seeking a Physical Therapist to deliver outpatient rehabilitation in home settings for older adults, including private residences and independent living facilities. You will assess, plan, and implement PT programs to restore function and promote independence.

The role emphasizes training patients and caregivers, coordinating with the rehab team, and maintaining high-quality services in a compassionate home environment. A PT license and relevant degrees are required.

Qualifications

• Must hold a valid Physical Therapy license in practice states.
• Master’s degree in Physical Therapy (or equivalent).
• Bachelor’s degree in Physical Therapy with 5 years of PT experience or equivalent.
• U.S. driver’s license or reliable transportation to patients’ homes.

Responsibilities

• Provide outpatient rehabilitation services in in-home settings for older adults.
• Develop and implement individualized therapy programs.
• Instruct and train patients and caregivers in required skills.
• Coordinate with rehabilitation team to address comprehensive patient needs.
• Maintain documentation and ensure high-quality, compassionate care.
• Supervise PT Assistants and other staff as needed.
