Job summary

TheraCare is seeking a licensed Physical Therapist (PT) for a Full-Time home health opportunity in Grayson & Cooke County, TX. Provide in-home PT services to adults and seniors, develop Plans of Care, and collaborate with an interdisciplinary team to improve mobility and independence.

Qualifications

• DPT from an APTA-accredited program.
• Active TX PT license or eligible Compact Licensure.
• 1 year Home Health experience required.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform home health PT evaluations and re-evaluations, including OASIS assessments.
• Develop and update individualized Plans of Care based on goals and physician orders.
• Provide skilled PT treatment for orthopedic, neurological, cardiac, post-surgical conditions.
• Educate patients and caregivers on exercises, safety, and use of equipment.
• Monitor progress and modify treatment plans; document in EMR.
• Collaborate with physicians, nurses, case managers, PTAs, and home health aides.
• Supervise PTAs and home health aides as per state regulations.
