Job summary

TheraCare Home Health is seeking a licensed Physical Therapist (PT) to provide in-home physical therapy across Grayson & Cooke County, TX. This full-time role offers flexible daytime scheduling and a one-on-one setting delivering compassionate care in patients' homes.

You will perform comprehensive evaluations, design and update Plans of Care, and deliver evidence-based interventions to improve mobility, balance, strength, and safety, while coordinating with the care team and documenting in the

Qualifications

• Doctorate in Physical Therapy from an APTA-accredited program.
• Active Physical Therapist license in Texas or eligible Compact Licensure.
• Home Health experience (1 year required).
• Current CPR certification.
• Knowledge of OASIS and home health regulations preferred.
• Strong communication, clinical judgment, and time-management skills.

Responsibilities

• Perform home health PT evaluations and re-evaluations, including OASIS assessments.
• Develop and update individualized Plans of Care based on goals and physician orders.
• Provide skilled PT treatment for orthopedic, neurological, cardiac, post-surgical conditions.
• Deliver interventions focused on mobility, gait, balance, and independence.
• Educate patients and caregivers on home exercise programs and safety.
• Document accurately in the EMR at the point of care and ensure compliance.
• Collaborate with physicians, nurses, case managers, PTAs, and home health staff.
• Supervise PTAs and aides per state regulations and agency policy.
• Participate in case conferences, discharge planning, and care coordination.
