Qualifications

• A current Pennsylvania Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association approved program.

• Solid computer skills; EMR experience preferred.

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, balance, strength, safety).
• Develop and implement individualized care plans including manual therapy and gait training.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care and daily living activities.
• Document observations and interventions using a touch pad tablet.
