Job summary

BAYADA Home Health Care, Inc. is seeking a part-time Home Health Physical Therapist (PT) for outpatient visits in the Media/Chester area of Pennsylvania. The role focuses on one-on-one care in adult and geriatric populations within Independent and Assisted Living communities.

The PT will join a multidisciplinary team delivering rehabilitative care at clients’ homes, with emphasis on safety, quality, and compassionate care. Strong clinical documentation skills and EMR experience are preferred.

Qualifications

• Current Pennsylvania PT license required.
• Graduate from an program approved by the American Physical Therapy Association or the AMA Committee on Allied Health Education and Accreditation.
• Ability to work independently with strong interpersonal, organizational, and problem-solving skills.

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, balance, strength, endurance).
• Develop and implement individualized care plans with manual therapy exercises and gait training.
• Revise care plans in collaboration with nursing and multidisciplinary team.
• Educate clients and families on rehabilitative care for independent living.
• Document observations and evaluations in EMR using a touchpad tablet.
