Job summary

BAYADA Home Health Care, Inc. is seeking a part-time Home Health Physical Therapist to perform outpatient visits in Swarthmore, PA, serving adult and geriatric clients in independent and assisted living communities.

You will work as part of a multidisciplinary team, documenting care plans, and delivering one-on-one therapy focused on quality and patient independence. Pennsylvania PT license required; strong EMR skills preferred.

Qualifications

• A PT license is required and must be current in Pennsylvania.
• Independent work ability and strong time management.
• Excellent interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, gait, strength, balance, and safety.
• Develop and implement individualized care plans with manual therapy, gait training, balance and other interventions.
• Assess and revise the PT care plan and coordinate with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care to promote health, safety and independence.
• Accurately document observations, interventions and evaluations using EMR on a tablet.
