BAYADA Home Health Care in Holmes, Pennsylvania is seeking a part-time Home Health Physical Therapist to deliver outpatient home health visits for senior living clients in the Media, PA area.

As a PT with BAYADA, you will work within a multidisciplinary team to develop and implement individualized care plans, conduct assessments, educate clients and families, document care using EMR, and help patients remain safely at home. A current PA PT license and strong time-management are required.

Formación

• PA PT license required.
• Graduation from a program approved by the American Physical Therapy Association or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, balance, strength).
• Develop and implement individualized care plans including therapeutic exercises and gait training.
• continually assess and revise the care plan in collaboration with nursing.
• Educate clients and families on rehabilitative care and home safety.
• Accurately document observations and interventions in EMR.
