BAYADA Home Health Care, Inc. is seeking a part-time Home Health Physical Therapist to perform outpatient home health visits for our senior living office in the Media, PA area.

The role offers one-on-one patient care with a focus on quality over quantity and opportunities to grow outpatient skills. You will be part of a multidisciplinary team delivering rehabilitative care in clients’ homes, with a commitment to safety, PPE, and infection control protocols.

Formación

• Current Pennsylvania Physical Therapist license.
• Graduation from a program approved by the American Physical Therapy Association (APTA) or AMA committee, as shown by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, transfer, balance, strength, endurance).
• Develop and implement individualized care plans (manual therapy, gait training, balance).
• Continuously assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and family in rehabilitative care and independence.
• Document observations and interventions using a touchpad tablet.
