Job summary

BAYADA Home Health Care, Inc. is seeking a part-time Home Health Physical Therapist in Aston, Pennsylvania to provide outpatient home health visits for seniors in Independent and Assisted Living communities in Media, PA.

This role offers flexible one-on-one patient care and a supportive team environment, emphasizing quality of care and professional growth. The PT will assess, plan, implement, and monitor rehabilitation programs, documenting progress and coordinating with multidisciplinary team

Qualifications

• Licensed Physical Therapist in Pennsylvania (PA)
• Graduation from APTA/AMA-accredited PT program
• Ability to work independently and manage time effectively

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements related to mobility, balance, and gait.
• Develop and implement individualized care plans including exercises and gait training.
• Continually assess and revise the PT care plan in coordination with nursing.
• Educate clients and family members in rehabilitative care and safety.
• Document observations and interventions in EMR with accuracy.
