Job summary

Beyond Home Health Care in New Jersey seeks a licensed Physical Therapist to provide in-home and community-based PT services to adults with developmental disabilities. You will evaluate mobility, strength, balance, gait, and functional movement, and develop personalized interventions to maximize independence within NJ DDD guidelines.

This contract/per-diem role offers flexible scheduling, home-based visits across the state, mileage reimbursement, and weekly pay.

Qualifications

• Active NJ PT license required.
• CAPTE-accredited PT program (DPT) required.
• Minimum 1 year clinical experience preferred; new grads considered.
• Current CPR/BLS certification.
• Clean background checks including fingerprinting per NJ DDD requirements.
• Reliable transportation and NJ driver’s license with active auto insurance.
• Ability to pass DDD background checks (CRoff).

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one PT services in home and community settings.
• Document sessions per DDD standards with notes and progress reports.
• Collaborate with coordinators, families, and interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce goals.
• Adhere to DDD policies and HIPAA confidentiality standards.
• Maintain billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle's/Komninos' Laws).
