Job summary

Beyond Home Health Care, a NJ DDD-approved provider, seeks a licensed Physical Therapist to deliver in-home and community-based PT services to adults with developmental disabilities across New Jersey.

The role focuses on evaluating mobility, strength, balance, gait, and functional movement, while aligning with ISP goals. Per diem/contract arrangement offers flexible scheduling and mileage support.

Qualifications

• Current NJ PT license required.
• Graduate from CAPTE-accredited PT program (DPT or equivalent).
• Minimum 1 year clinical experience preferred; new grads considered.

Responsibilities

• Conduct comprehensive in-home evaluations and develop individualized treatment plans.
• Deliver one-on-one PT services in home and community settings.
• Document services per DDD standards within required timeframes.
• Collaborate with coordinators, families, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals.
