Job summary

Beyond Home Health Care is a NJ DDD-approved provider delivering in-home physical therapy to adults with developmental disabilities across New Jersey. The PT will evaluate mobility, strength, balance, gait, and functional movement, delivering person-centered interventions to maximize independence and participation.

We offer per diem/contract roles with flexible scheduling, mileage support, and rapid weekly pay.

Qualifications

• Current New Jersey license as a Physical Therapist (PT) issued by the NJ State Board of Physical Therapy Examiners.
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy services in homes and community settings.
• Document services per DDD standards with session notes, progress notes, and outcome measures.
