Job summary

Beyond Home Health Care in New Jersey is seeking a licensed Physical Therapist to provide in-home and community-based PT services to adults with developmental disabilities. You will evaluate mobility, strength, balance, gait, and functional movement, delivering person-centered interventions to maximize independence and participation.

This per diem/contract role offers flexible scheduling, mileage considerations, direct deposit, and supportive leadership, with compensation of $76–$82 per hour.

Qualifications

• Active New Jersey license as a Physical Therapist.
• Graduate of a CAPTE-accredited physical therapy program (DPT or equivalent).
• Minimum of one year of clinical experience preferred; new graduates considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation and valid NJ driver’s license.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and goals.
• Deliver one-on-one therapy services in the participant's home and community settings.
• Document services in compliance with DDD standards, including session notes, progress notes, and outcome measures, within required timeframes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce therapeutic goals between sessions.
• Adhere to all DDD policies, including HIPAA confidentiality standards.
• Maintain accurate billing records consistent with DDD rates and authorizations.
• Complete required DDD-mandated trainings.
